This policy explains how the Dental Practice Sven Bival, doctor of dental medicine, specialist in oral surgery (hereinafter: the \u201cPractice\u201d, \u201cwe\u201d) processes the personal data of patients and visitors to the dental-bival.eu website, in accordance with the General Data Protection Regulation (EU) 2016\/679 (\u201cGDPR\u201d) and the Act on the Implementation of the General Data Protection Regulation.<\/p>\n<p>We do not sell your personal data or hand it over to unauthorised third parties, including for the promotional purposes of third parties.<\/p>\n<h2>1. Patient data<\/h2>\n<p>We collect patients' personal data primarily to provide dental services, namely for:<\/p>\n<ul>\n<li>dental examination and diagnosis,<\/li>\n<li>treatment and procedures,<\/li>\n<li>preparing quotes and treatment plans,<\/li>\n<li>taking impressions for prosthetic restorations,<\/li>\n<li>keeping medical records and records during procedures,<\/li>\n<li>protecting healthcare workers who come into contact with the patient,<\/li>\n<li>monitoring patient satisfaction.<\/li>\n<\/ul>\n<p>For these purposes we process: first and last name, address, date of birth, OIB, telephone, mobile phone, email address, name, address and telephone number of the next of kin, data on past and current health condition, diagnoses, treatments and therapies performed, dental impressions, X-ray images, and intraoral and extraoral photographs before, during and after procedures.<\/p>\n<p>We collect, process and store this data on the basis of law. A doctor of dental medicine is obliged to keep as professional secrecy everything learned about a patient's health condition, even after the patient's death. Disclosure of professional secrecy is permitted only with the patient's consent, in accordance with the Code of Dental Medical Ethics and Deontology and the law, for example: for scientific purposes without revealing identity and with written consent, for statistical processing without revealing identity, to a health insurance company with which the patient has a contract, and to the competent authorities where legally required.<\/p>\n<p>If we ask for your consent for other purposes (e.g. promotional), we will state the retention period on the consent itself. These processing activities are not carried out through the website. Please do not send health data through the website.<\/p>\n<h2>3. Data about your visit to the website<\/h2>\n<h3>3.1. Contact form and contact by email or telephone<\/h3>\n<p>When you send us an enquiry through the form, we process your first and last name, email address, topic and message text, solely to answer your enquiry. The legal basis is your consent, or our legitimate interest in answering the enquiry (Art. 6(1)(a) and (f) GDPR). Messages arrive in the Practice's mailbox and we keep them only as long as necessary to reply and manage the communication. If an enquiry results in treatment, further processing follows the rules in section 2.<\/p>\n<h3>3.2. Analytics and cookies<\/h3>\n<p>With your consent we use Google Tag Manager and Google Analytics (Google Ireland Limited) to understand how the website is used (pages visited, visit duration, approximate location, device type). Without your consent these tools are not started. You can change or withdraw your consent at any time. More in the <a href=\"\/en\/politika-kolacica\/\">cookie policy<\/a>.<\/p>\n<h3>3.3. Server logs<\/h3>\n<p>The website server automatically records technical access data (IP address, time of access, requested URL, browser) for security and the correct operation of the website. The legal basis is legitimate interest (Art. 6(1)(f) GDPR). We keep the logs only as long as necessary for those purposes.<\/p>\n<h3>3.4. Third-party content<\/h3>\n<p>On the Contact page we display a Google Maps map. The map loads automatically when that page is opened, so Google Ireland Limited receives technical data about your visit (e.g. IP address and browser data) and may set its own cookies in accordance with its own privacy policy. If you do not want this, do not open the Contact page or contact us by telephone or email.<\/p>\n<p>YouTube videos on the website play only after you click play and are loaded through YouTube's enhanced-privacy version (youtube-nocookie.com). Video preview images are downloaded from YouTube (Google), so technical data about the visit may be transmitted to Google when a page with a video is opened.<\/p>\n<h2>4. Who we share data with<\/h2>\n<p>The data you give us may, for the purpose of providing our services, be passed to our processors: accounting (invoice data), the laboratory (for making impressions and prosthetic work), and providers of technical services for the website, hosting and email, and to Google for analytics, maps and video, subject to contractual data protection. We may also disclose data to competent authorities where legally required. Google may process data outside the European Economic Area with appropriate safeguards (adequacy decision or standard contractual clauses).<\/p>\n<h2>5. How long we keep data<\/h2>\n<p>We do not keep data longer than necessary for the purpose for which it was collected. A doctor of dental medicine is obliged to keep data on a patient's treatment for ten years after treatment ends, and after that to act in accordance with the regulations on the retention of documentation. We are required by law to keep data contained in invoices and on financial transactions for 11 years. The retention period for data collected on the basis of consent is stated on each individual consent.<\/p>\n<h2>6. Your rights<\/h2>\n<p>You have the right:<\/p>\n<ul>\n<li>to information about the processing of your personal data,<\/li>\n<li>to access personal data about yourself,<\/li>\n<li>to request correction of inaccurate, imprecise or incomplete data,<\/li>\n<li>to request erasure of data when it is no longer needed or the processing is unlawful,<\/li>\n<li>to object to processing for marketing purposes or on grounds relating to your particular situation,<\/li>\n<li>to request restriction of processing in specific cases,<\/li>\n<li>to receive your data in a machine-readable format and transmit it to another controller (data portability),<\/li>\n<li>to request that decisions based on automated processing be made by natural persons, and to express your point of view and contest the decision,<\/li>\n<li>to withdraw consent at any time, without affecting the lawfulness of processing before withdrawal.<\/li>\n<\/ul>\n<p>You can send requests to <a href=\"mailto:info@dental-bival.eu\">info@dental-bival.eu<\/a>, call us on 01 4666 775 or write to us at the Practice's address. Security and changes<\/h2>\n<p>The website uses an encrypted (HTTPS) connection, and we protect data with technical and organisational measures.
